Rules for CLR files

I thought for sure there would be a sticky about this, but...

Can someone point me to a set of rules for creating color schemes (.CLR files)? I understand the RGB values. But what determines how the RGB values are assigned to map elements? Are the remarks after "//" on each line comments, or do they assign the color to the map element described in the comment? If the remarks are simply comments, what determines the assignment? Is it the sequencing of RGB values within the files, or what? Are there particular keywords that need to be used in the comments to effect an assignment? What are the rules?

This is great. Now I know the assignments for the first 71 lines (documented in the sample file, among others).

One more question: I notice that some .CLR files have additional lines with descriptors all starting with "EFace", for example, "EFaceTypeBuilding". Are these supported by all TT devices?
